The Case History provides the doctors with much needed background information on all prospective new patients.
The Pediatric Case History is designed to provide the doctors with all the necessary information pertaining to patients under the age of 5.
All patients presenting with a primary complaint of neck pain are asked to complete the Neck Pain Index.
All patients presenting with a primary complaint of lower back pain are asked to complete the Low Back Questionnaire.
